She's still says it was the most embarrassing thing to happen to her, so I think it was real. Plus, the MTV cameras missed it, while the ABC cameras caught it.

I think it was real, no way would Whitney herself stage something like that. She was so upset after too. But, what do you mean the MTV cameras didn't catch it? I may have to watch again but I thought that it was shown on the show (the exact moment she fell).

If you noticed, the MTV cameras did not catch Whitney's fall. They caught Lauren's reaction but not Whitney's fall. MTV either begged for or bought the footage from Disney/ABC/Good Morning America.
