Game of Thrones : the books

I've watched only the first season so far and I havnt read the books yet.Since the first book of the series is named Game of Thrones does that mean the whole tv series take place during the first book only or the whole series?

The 1st season is the first book only.
Seasons 3 & 4 are book 3.
Season 5 & 6 are books 4, 5 and beyond. Book 4 is parallel to the first part of book 5.
Book 6 is coming out in five hours, or maybe this year, or not.
