Die Hard with a Vengeance: McClane drives in Central park then over this 'rock' ramp to get on the street. When the car lands on the street, one of the back tires clearly falls off. Then it's magically back on the car again, because they can drive just fine after that. It's incredibly obvious.

I always thought the shot you're referring to was showing the T1000 having "grown" a 3rd arm to control the helicopter while using 2 hands to reload his gun.

In Hidalgo when the villain sends out some cheetahs after the hero they are clearly CGI why was it done? they didn't get killed, in Gremlins when Stripe is carried away by the chainsaw you can see the track that is pulling him, in Jaws The Revenge you can clearly see a rod in the shark's midsection after it kills Jake, and in Happy Gilmore in the scene where the hockey officials are talking about Happy he breaks the window they are in front of seconds later the window is intact.

In "You Only Live Twice" Aki's white Toyota 2000 roadster suddenly switches from a right hand drive car to a left hand drive and back again. Also while she and Bond are speeding along a highway, the speedometer and tach are on dead zero.

Oh yeah I forgot the most obvious one of all that wasn't included on this list, In Star Wars A New Hope when the stormtroopers march through a sliding door one of them clearly bumps his head.

I think the Star Wars one is legendary and when Lucasfilm cut it out of the 1997 re-release there were so many complaints they were forced to put it back in, along with a comedy sound effect for the dvd release, to keep fans happy.
Worst goofs? I think the scene with Paul Newman having an argument in the middle of the street in Slapshot (1977) is quite amusing. The newspaper under his arm keeps moving from armpit to hand, to other hand, depending on which camera shot is used. Also check out the appalling blue screen use in Jaws 3D, the whole underwater viewing area set just looks so incredibly fake. Or how about Highlander 2? Its bad enough if you have scenes of people flying through the air with the stunt wires obviously visible but some of those scenes were carelessly also left in the films promotional trailers. Not a smart move.
A more recent one is the obvious CGI James Bond in the paragliding scene in Die another Day. He just looked so fake and everybody knew it was CGI. Come to that the Scorpion King in The Mummy Returns looked really fake too, especially the face. It was so unconvincing it looked like a plasticine model.

And in the Bond film Diamonds Are Forever , the scene in the convertible Mustang. It begins a two-wheel skid on the left two wheels and then is shown on the right two wheels after coming out of a tunnel!

In First Blood, Rambo steals an empty army truck, you can see the back window. Later he opens it up and it's loaded to the tailgate with weapons. Never mind regulations about transporting weapons and ammo seperately, or travelling alone.
Also in Game of Death, the cutout of Bruce Lee on the mirror covering the stand-ins face.

The entire Schwarzeneggar movie Commando is a Goof. When he is holding Sully over the cliff with one arm (yeah right) you can clearly see a line tied to his pants holding him up (no Arnold's massive arms!). Just previous to that, there is a chase scene with a yellow Porsche. It is side swiped, bmped and eventually crashed and rolled. But when Arnold turns it over and drives away, it is a brand new car.
This whole movie is so bad that it is funny to watch and pick out stuff like this.

Kirk is standing in an airlock preparing to head out to rescue Spock. In the wide exterior shot, they totally forgot to composite in the ship around him. What we see is a man in a space suit standing in a small airlock that's on top of lots of scaffolding.
What's frightening is I had to read about this goof before I ever noticed it while viewing. Now the scaffolding is all I ever see. I suppose it's sort of like the snake's reflection in Temple of Doom. Once you know--it's all you see.

I like the crew-members who appear in Braveheart, Robin Hood: Prince of Thieves and Gladiator - all in very 20th/21st dress. But my favourite has to be the extra during a street scene in Marty who turns and waves at the camera (2nd place is the thumbs up in Wild Things)

In the Dirty Harry sequel Sudden Impact, the scene when Harry is doing target practice in the woods and his cop buddy Horace comes along to greet him. As Horace gets out of his car, you can clearly see the reflection of the camera man on the car door window.

In "Steel Magnolia's" . . . Years go by and Julia Roberts brothers in the film are portrayed by the same actors. They never age, at all and never grow another inch taller -- and I think they wear the same clothes throughout, too! Not sure about it ... but when they are seen, they do not change, grow, etc.
In "Robin Hood: Prince of Thieves" Kevin Costner has a California accent even though he claims to "be home once again" in England!

The first one I have was an obvious joke, but I'll mention it and maybe get a few chuckles: In "I'm Gonna Git You Sucka" theres a scene, I think involving a Wayan's brother, where a women with long hair flips around, and when they show the face in quick succession, it's obvisously a male (stunt double), but the man also has a black mustache, which shows up quite well, causing the audience to start laughing at its absurdity.
The bullet holes already being there in Pulp Fiction, before the young thug starts blasting at the Samuel L Jackson and John Travolta characters is the best I can come up with.

Well, this isn't the most obvious goof of all time, but it's what I notice everytime I watch the movie, even the first time, I think, and it's what I can think of right now. In Stardust, at the end, when the witch shatters the glass around Tristan and Yvaine, her hair goes flying everywhere, all up on her head and is very messy. For two shots I think it's like this, but when it's over the camera goes back to her and her hair is smooth and hanging down beside her face instead of all over her head. It bugs me. Why comb her hair for the third shot when it's going to be so obvious? I notice it every time.

Not completly obvious but in Transformers (2007), after Bumblebee "breaks down" and Mikaela opens the door and exits, she leaves it open. Latter, when she walks off and Sam tries the engine, the door is clearly still open, then once the engine has started and Sam gets out to close the hood, the door is closed. It could be argued that Bee closed it but there isn't time between shots.

When characters get into cars/truks/buses and drive them they constantly jerk the steering wheel left to right; right to left for no logical reason.
Reminds me of the way Jack Nicholson drives a VW at the beginning of The Shining, lol.

I would have to say in Terminator 3 all the crap that Toyota Tundra went through during the chase scene and there was very little damage done to it.

I've read some of the goofs that people have posted but I have one that is so obvious that it is hilarious. In a very low budget movie called Return of the Living Dead 3 a guy get's his arm ripped off by a zombie and after he turns into a zombie it is extremely obvious that he's holding his arms behind his back. I mean that is so obvious that I noticed it like five times in different scenes.

This is a tv show, but currently the only goof I can think of
In the season 3 episode "Bad Girls" of Buffy the Vampire Slayer, when Buffy is in a lesson, and Faith comes to her classroom window (which is open) on the outside of the building. She's leaning in through the window, and you can clearly see that the grass and bushes outside are covered in snow (it could have been fake snow as another episode around the time of this one had snow in it). You can see some green grass sticking through the snow etc. After this, Buffy decides to ditch the class, so she sneaks out through the window and then when she's outside you can see that there's no snow whatsoever.

In the Fifth Element when Zorg makes the payphone blow up while he's talking to one of his henchmen. After the explosion you can clearly see the mannequin stand-in for the actor still upright!

Oh yeah I remembered one that makes me laugh so much every time! It's in The Poseidon Adventure (1972). When Roddy McDowalls' character Acres falls to his death, firstly you see the shot of Acres falling, then there's a shot of Ernest Borgnines' character looking into the water below in shock, but as EBs' character looks down beneath him, you can clearly see the manequin of Acres still attached to the ladder. So basically Acres has just fallen into the water, but you can still see him (manequin) on the ladder.

In Twister, when they are chasing the f-5 tornado towards the end. The unloader of the combine comes right through the passenger window, but in another shot, the window is just fine.

In Twister, there is a scene with the main guy and girl together in the pick up. The guy tells the girl to tighten up her seat belt as he pulls the strap across her chest down. That action will only cause a seat belt to loosen up.
In Jurassic Park when the T-rex is chasing the Jeep, the driver looks at the left side mirror. Then the shot changes to that mirror where the phrase "OBJECTS IN MIRROR ARE CLOSER THAN THEY APPEAR" is printed on it. That phrase should only appear on the right side mirror.

Other than anything from Ed Wood (Plan 9 obviously has the most obvious goofs, mostly for the aforementioned day-night transitions), I would say the red carpet disappearing in Rocky Horror Picture Show during Columbia's dance in the middle of the Time Warp.

In the TV version of "Gypsy" starring Bette Middler, every time the camera angle changes during the first scene at the Uncle Jocko rehearsal, poor little Chowsie (the dog) is in a different spot! In less than a second, it's as if the little guy has hopped from Bette's right arm to her left, and back again, and again, and again!

In the movie Without a Paddle when they are in the pot fields you can see someone crawling on the ground behind them.
