Still trying to get my head round it.The doctor tells him he'll be "Incarcerated",if he doesn't get re-implanted.And in the end he see's his wife/girlfriend just as a projection,is that part of the punishment?She never really existed in the first place and just used as a sexual fantasy for the soldiers?

The way I understood it, he chose the re-implant.

I think it's meant to show that not only are their minds manipulated, so they have no qualms about what they do at the front, but the soldiers are also given a fake home front too, so they don't ponder about that either.

I don't think the soldiers live(d) different lives than the villagers you saw in the beginning.

I'm thinking that she never existed in the first place. The hint that I got was that when the psychiatrist gives him something to sleep "really well" and he has that insane dream that's markedly different from the more basic tantilization of his other dreams.

So the manipulation of the soldiers' dreams is probably a way to keep the soldiers morale up, basically a "this is what you're fighting for" bit of propaganda that plays while they sleep.
