I'm not sure but I read somewhere that they were conjoined twins
I have two theories:
Conjoined at birth, separated, leaving a scar.
Or
Some weird burn mark, 'pact' to show how close they are.
If the second one is true, I suspect the parents' reaction finding Isabelle and Theo (forgetting about Matthew here for a second) naked together in a tent wouldn't seem so crazy, hence their "lets leave money and get out of here quickly" reaction.
They've already witnessed something similar beforehand.
